To get Form-V, your Principal Employer (From whom he has been issued Work Order) has to take Registration under Contract Labour (R & A) Act, 1970 from the Registering Authority. The process is available in this portal in the following links. https://wblc.gov.in → E-Services → Guideline for applying licence under Contract Labour (R & A) Act, 1970. Then Principal Employer will provide the Form-V(s) to all contractors.
